  • Connect Physical Therapy & Wellness operates as a cash-based clinic and is an out of network provider, meaning we do not accept direct insurance payments. However, we can provide a detailed invoice (superbill) for you to submit to your insurance provider for potential reimbursement.

    For clients with Medicare Part B, we can assist as a non-participating provider. You will pay for services upfront, and we will submit the claim to Medicare on your behalf. Reimbursement is typically around 75% of the cost, depending on your specific Medicare plan.

  • Being an out-of-network provider means that a healthcare provider, like Connect Physical Therapy & Wellness, does not have a contract or direct agreement with your insurance company. As a result, the provider does not accept the insurance company’s rates or terms for payment.

    If you choose to submit a superbill to your insurance company, reimbursement will depend on your plan’s out-of-network coverage. Reimbursement rates can vary, and some insurance plans may cover only a portion of the costs or none at all.

  • We are located in Suite 104 inside of Always in Motion Gym located at 6930 Warner Ave., Huntington Beach, CA. This is a gym for people 50 years of age and better.  We have a private treatment room as well as access to the gym space with a treadmill and harness system, overground harness system, cardiovascular gym equipment and strengthening equipment.   • Yes, we do home visits. There is a travel fee of $40 associated with traveling to your home. You may choose to do an evaluation session or treatment sessions in your home to optimize your mobility and get feedback on how you are performing exercises and activities in your home. You may also choose to have us come to your home out of convenience in scheduling. You may choose to have home sessions as it is difficult to get to a gym or clinic. Whatever the reason, Connect Physical Therapy and Wellness aims to meet you where it serves you best.
